Student Insurance coverage

COVERAGE FOR ACADEMIC YEAR 2017/2018

 
Period of insurance coverage : 1 July 2017 to 30 June 2018
 

Eligibility

  • Full-time local and international Undergraduates*
  • Full-time and part-time local and international Graduate students, and Non-graduating students*

* Cessation of the student insurance coverage is based on a student's graduation date. Please note that the graduation date is NOT the same as commencement date. 

 

Student Insurance Coverage

All eligible students are covered by the following products:

  1. Group Clinical Outpatient (GP)
  2. Group Specialist Outpatient (SOP)
  3. Group Hospital & Surgical (GHS)
  4. Group Personal Accident (GPA)

 

Group Clinical Outpatient Consultation (GP)

  • Your visit to “The Clinic At Campus Pte Ltd” and the clinics in our PANEL LISTING (approximately 200 clinics), will be cashless when you produce your SMU student card upon registration.

  • No payment is required at these clinics for consultation and standard medication for an insurance covered medical condition during normal clinic operating hours. However, you may have to top-up for non-standard medicine and surcharge for visits after normal clinic operating hours.

Group Specialist Outpatient Consultation (SOP)

  • Outpatient specialist consultation (includes physiotherapy), X-ray and diagnostic services are reimbursable, on the condition that:

o    The treatment is at a Government/Restructured Specialist Clinic.

o    The treatment by specialist is referred by GP/A&E; OR

o    It is a follow-up treatment as referred by SMU's previous appointed medical provider

  • Covers hospitalization expenses incurred in Singapore Government/Restructured hospitals:

Group Personal Accident (GPA)

  • Provides 24 hours worldwide cover

  • Physiotherapy & Chiropractor have to be referred by General Practitioner (GP)

  • Outpatient treatment is reimbursable via e-claims submission

  • In event of hospitalization, claim forms and original hospitalization invoices would need to be manually submitted to Student Services Hub (SSH)

Claims Submission (Undergraduates and Post-graduates ONLY)

  • Please go to https://eben.aia.com.sg for registration before performing your first claim submission.

  • To submit a claim, please follow the step by step guide here.

  • All claims can be submitted via the web-based E-benefit Portal, except for Group Hospitalization and Surgical (GHS) claims. This portal allows you to:

o    Submit outpatient claims online

o    View claims and status

o    Generate E-card for your visit to General Practitioner (GP) panel clinics in the absence of SMU Student card

o    GP Clinic Locator

  • For manual submission of Group Hospitalization and Surgical (GHS) claim, please download and complete the Hospitalization Claim Form and submit to Student Services Hub (SSH).  

  • All claims with required supporting documents must be submitted within 30 days after each treatment. For Specialist Claims, you would need to submit the referral letter from panel GP.

  • All original invoices/receipts must be kept for 6 months and be submitted upon request. (applicable for e-submission claims only) 

 

Claims Submission by Non-graduating (Exchange students ONLY)

  • All claims MUST be manually submitted to Student Services Hub (SSH). 

  •  Please download and complete the necessary forms below:

o    Outpatient Claim Form

o    Hospitalization Claim Form

  • All claims with required supporting documents (ie. Original invoices/receipts) must be submitted within 30 days after each treatment. For Specialist Claims, you would need to submit the referral letter from panel GP.

  • In the event that you are unable to submit the claims within the above stipulated timeframe, please inform steveloh_smu@aia.com.sg or aiasmu@alliancemedinet.com within 30 days after each treatment and follow up with your supporting documents when available.
