Student Insurance coverage

COVERAGE FOR ACADEMIC YEAR 2016/2017

 
Period : 1 July 2016 to 30 June 2017
 
Eligibility :
  • Full-time local and international undergraduate, exchange and bridging students
  • Full-time and part-time local and international graduate students
 
Group Personal Accident Coverage
  • Accidental Death – S$35,000 per student
  • Accidental Permanent Disablement – up to S$52,500 per student
  • Accident Medical Reimbursement – S$5,000 per student
  • Registered Chinese Physician, Physiotherapy & Chiropractor Reimbursement – S$750 per student within Accident Medical Reimbursement of S$5,000 limit (referral letter from a doctor s required for Physiotheraphy and Chiroprator treatment) 
  • 24-hour worldwide coverage (coverage and exclusion details)
 

Claim Procedure

  • You should submit the claim form within 30 days after each treatment.
  • If you are unable to submit the form within the above-mentioned period, please send a notice of claim to AXA / MYCG via email (smu@mycg.com.sg) within 30 days after each treatment and follow up with your supporting documents when available.
  • You must obtain a referral letter from your General Practitioner (GP) for Specialist Treatment. X-ray and Diagnostic Services must be recommended by a doctor.
  • You have to submit your duly completed claim form, all original bills/receipts, hospital discharge summary (for hospitalisation, surgery or day surgery done at government or restructured hospitals), written test reports and the aforesaid referral letter to the Office of Student Life.
  • Once AXA receives all the relevant documents, it will process your claims. You will be reimbursed for the approved claims, which will be credited into your bank account, within one to two months.
 

Outpatient GP Consultation Coverage

  • You will be reimbursed for your visits to The Clinic@Campus, approximately 200 GP panel clinics and the Government polyclinics.   
  • You can claim for consultation, standard medication and simple lab tests and x-ray (coverage and exclusion details).  
  • You will have to top-up for non-standard medication (original/branded/patented medicine) as this is not covered under the insurance.

For Referrals to a Singapore Government/Restructured Hospital for Specialist Treatment and/or Hospitalisation/Surgery:

  •  If you are referred by a Panel GP Clinic, you are considered a Private patient;
  • To qualify as a subsidised patient, you must be a Singaporean or Singapore PR, referred by a Polyclinic and without reference to a Specialist by name.

 

The Clinic@Campus and Panel Clinics 

  • You have to show your SMU student card upon registration.
  • You will not be required to make payment at the clinic for consultation and standard medication for a medical condition that is covered by the insurance. You may have to pay for non-standard medicine.
  • If you forget to bring your SMU student card to the clinic, please contact MYCG at 6476 3829 / 9762 2062 and they will verify your status as a SMU student with the clinic. Alternatively, you can pay first and submit a claim for reimbursement within 30 days after treatment.

 

Polyclinics and Hospital A&E Department

  • Please pay first and submit the claim for reimbursement.
  • You should submit the claim form within 30 days after each treatment.
  • If you are unable to submit the form within the above-mentioned period, please send a notice of claim to AXA/MYCG (smu@mycg.com.sg) within 30 days after each treatment and follow up with your supporting documents when available.
  • You have to submit your duly completed claim form, all original bills/receipts and written test reports to the Office of Student Life.
  • Once AXA receives all the relevant documents, it will process your claims. You will be reimbursed for the approved claims, which will be credited into your bank account, within one to two months.

 

Outpatient Specialist Consultation Coverage

Outpatient specialist consultation, prescribed medication and  diagnostic services and physiotherapy recommended by a Specialist at a Singapore Government/Restructured Hospital/Outpatient Specialist Clinic are reimbursable on the condition that:

  •  you are referred to the Specialist by a GP/A&E; or 
  •  it is a follow-up treatment as referred by SMU's previous appointed medical provider (for follow-up treatment within 1 year of the last visit). A new referral is required if the last visit to the Specialist was more than 1 year ago.

Please note that treatment by Private Specialists/Physiotherapists is not covered. Physiotherapy done at the Clinic@Campus is not covered under this benefit.

Hospitalisation and Surgical Coverage

Hospitalisation Expenses Incurred in Government or Restructured Hospitals

For students who are Singaporean or Singapore Permanent Residents, please refer to these coverage and exclusion details
